Trailblazer Award

     

Does Your Practice Have a Rising Star? Please Nominate Your Peers for the Trailblazer Award.  

The Trailblazer Award is a prestigious recognition bestowed upon urologists or administrators who have significantly contributed to the field within the first ten years of their practice. This award acknowledges their exceptional achievements, innovation, leadership in urology, and commitment to advancing the field.

Recipients of the Trailblazer Award have demonstrated a pioneering spirit and contributions to their practice, breaking new ground in patient care, research, education, or administration. They have shown remarkable dedication to improving the lives of their patients, embracing cutting-edge technologies and techniques, and making valuable contributions to the community. Trailblazer Award recipients have exhibited exceptional leadership skills. They have demonstrated contributions to LUGPA through engagement or committee involvement. Groups and individuals may make nominations; however, self-nomination is not permitted.

ELIGIBILITY: 

  • Candidate must be part of a LUGPA group.
  • Candidate must be a physician or administrator in a medical practice for at least two years but less than ten years. 
  • Currently, APPs and other staff are not eligible.

SUBMISSION REQUIREMENTS

  • Cover letter explaining why you or your group practice supports the candidate.  
  • Candidate’s curriculum vitae

SUBMISSION DEADLINE No longer accepting submissions for 2024. Deadline was Aug. 30, 2024.  

AWARD NOTIFICATION: Award recipient(s) will be notified by late September.

AWARD RECOGNITION: The recipient(s) will be recognized at the LUGPA Annual Meeting in early November.
